Consultations

Cosmetic consultations include discussions about various elective cosmetic (non-insurance covered) procedures that we offer, for example laser, peel, filler, Botox, tumescent liposuction procedures. The consultation fee will then apply to any cosmetic procedure performed or product purchased. Medical dermatologic evaluation of skin lesions or medical advice and/or prescriptions for skin conditions are considered office visits, billable to insurance or directly to the patient. Prices for medical evaluations vary based on complexity. We are often able to address both cosmetic and medical concerns in a single visit. If we do so, the charges will be cumulative. If you need clarification, please review this with one of our staff members at the time of scheduling.

Need extra privacy? We can arrange to see you after hours in the office with minimal staff and no patients.

For ultimate privacy, in certain cases Dr. Guanche and or her team members can come to you. Cases are considered individually, and are subject to approval.

It is the patient's responsibility to confirm their insurance coverage and benefits, pertaining to medical dermatology care at Bella Skin Institute. Please note that cosmetic procedures are not billed to or covered by commercial insurance carriers.

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. Below are links to our patient information and medical forms. We have also included a privacy statement for you to review and keep for your records. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.

Co-payments must be rendered at the time of the visit. We accept VISA, MASTERCARD, AMERICAN EXPRESS, Debit Cards, Cash and Checks. We also offer Third party financing through CareCredit® which will allow patients to make payments interest free for 12 months.

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. There is a $100 cancellation fee for all providers, including aestheticians and brow technician
