Cosmetic consultations include discussions about various elective cosmetic (non-insurance covered) procedures that we offer, for example laser, peel, filler, Botox, tumescent liposuction procedures. The consultation fee will then apply to any cosmetic procedure performed or product purchased. Medical dermatologic evaluation of skin lesions or medical advice and/or prescriptions for skin conditions are considered office visits, billable to insurance or directly to the patient. Prices for medical evaluations vary based on complexity. We are often able to address both cosmetic and medical concerns in a single visit. If we do so, the charges will be cumulative. If you need clarification, please review this with one of our staff members at the time of scheduling. Private In-Office After Hours Consultation $500 Need extra privacy? We can arrange to see you after hours in the office with minimal staff and no patients. Private In-Home Consultation $1000 For ultimate privacy, in certain cases Dr. Guanche and or her team members can come to you. Cases are considered individually, and are subject to approval. By filling out these forms ahead of time, you will save a significant amount of time during your visit. This will take you to Adobe's website and enable you to download the Acrobat program to your computer. Payment Policies Co-payments must be rendered at the time of the visit. We accept VISA, MASTERCARD, AMERICAN EXPRESS, Debit Cards, Cash and Checks. We also offer Third party financing through CareCredit® which will allow patients to make payments interest free for 12 months. Cancellations If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. There is a $25 cancellation fee for standard appointments, and a $50 fee for extended appointments.
